Coaching Session Agreement
Participants:
This client agreement is effective to the above date, between the client named above (referred to as “Client"), and Drew Eric Miller, (referred to as "Coach")
Type of Relationship:
The client enters into coaching with the understanding that he/she is responsible for making decisions and creating his/her own results. The client engages the coach for the purpose of providing a structure for identifying and moving towards the goals that are created by the client. The coach is to support the client’s development as it relates to any or all of the following: life skills, communication skills, business and professional objectives, networking skills, and/or interpersonal relationships. The coach has experience in such matters and agrees to render such services. Client also agrees not to hold the coach liable for any actions or results for adverse situations created as a direct result of a specific activity undertaken by the client.
Prior to Each Coaching Session:
The client is solely responsible for creating each coaching session. The client is encouraged to create an intention for the coaching session. This will engage the client in the coaching process prior to the actual coaching session and ensure concise conversation, building on clarity and transformation.
Communication:
The coach will make requests of the client as part of the coaching relationship. The client may say yes, decline, or counter–offer at any time. If the coach does or says something that upsets or feels uncomfortable to the client, the client is to bring it to the coach’s attention immediately. As coach, I promise to make the coaching experience beneficial for the client and to do what is necessary to ensure the client is satisfied.
Disclosure
: The coaching relationship is in no way to be construed as psychological counseling or any type of psychotherapy. It is further understood that the coach is not an employment agent, business manager, financial analyst or psychotherapist and has not promised, nor is he obligated, to provide services which include: procuring or attempting to procure any employment, business or sales for the client; performing any business management functions such as accounting services or advising on tax matters or investing funds; acting as a therapist, a counselor to provide psychoanalysis, group or behavioral therapy.
Retainer and Terms:
For maximum results and transformation a typical long term transformation occurs during 3-6 months of coaching. The monthly retainer selected below, includes four 30-45 minute scheduled coaching sessions a month. Telephone meetings will be the norm, although occasional face-to-face sessions may be available. Payments are due, in advance, to be received by the 1st of each month care of Drew Eric Miller at 1911 NE 8 Ct, Suite 120, Fort Lauderdale, FL 33304. Payments may be made by Check, or Credit Card (see payment authorization form).
Duration of agreement:
This is an individual coaching agreement of for one - three months depending on the client, and shall automatically renew for the same terms unless either party notifies the other, in writing, of its intent not to renew at least thirty (30) days prior to such end date. At anytime coaching can be complete without any further contracts or payments needed. A refund for remaining sessions can be disbursed. A full refund for all services can be disbursed at the coach’s discretion and coaching can be terminated. Coaching is 100% guaranteed.
Confidentiality:
All information (documented or verbal) that the client conveys to the coach is confidential and is not shared with any third party, unless the client chooses to share it. This confidentiality applies to any proprietary information the client shares with the coach.
The coach will not give out the name of any client (as a referral) without the client's consent. With the clients consent, coaching calls can be recorded for the benefit of the coach for personal growth and/or mentoring with another coach unless the client request no-recording. In addition, some recordings can be used for certification purposes with the International Coaching Federation, ICF—only with the clients consent. At any time a client can stop a recording if they wish to do so for privacy. Recordings will be kept for approximately 1 year after the final coaching session then destroyed. Personal identification is not necessary with the recordings. The client will be notified if the coaching session is to be recorded. At no time will a client be recorded without prior knowledge.
This confidentiality statement has the applicable state, providence or federal laws, and the clients own company policy (if applicable) allows. The coaching relationship is not privileged and, as such, the coach’s records on a client can be subpoenaed.
Rescheduling:
If the client finds it necessary to reschedule an appointment, please allow for at least 24–hour notice. If an emergency arises, notify the coach as soon as possible and the appointment will be rescheduled (See following appointment cancellation policy). If the client misses an appointment without any notice, it will not be rescheduled and is considered a billable coaching session. It is the client’s responsibility to keep appointments. If more than two (2) appointments are missed then coaching is subject to termination due to lack of participation.
Session Procedure:
The time of the coaching sessions and/or location will be determined as appointments are scheduled each month. When meeting via phone, the client agrees that they will initiate all scheduled calls and call the coach at

at the appointed time. If the coach will be at any other number, the client will be notified prior to the scheduled appointment time.
Appointment Cancellation Policy:
If the client needs to cancel an appointment, please allow for a 24–hour notice.
It is understood that there are times when an emergency will present itself (e.g., a summons to jury duty, a medical emergency, or family emergency). The coach will do his best to reschedule the sessions.
If the coach has a conflict and needs to change a scheduled appointment, he will always reschedule and make sure the client receives the appointments that have been paid for.
If the client has to change an appointment with 24 hour notice, the coach will make sure it is rescheduled.
If the client has to change an appointment with less than 24 hour notice, the coach will do what he can to reschedule within that month, if possible.
The preceding client agreement is intended to provide clarity, and establish the business framework that we will agree to work within. By submitting this online form the client agreed to the above coaching agreement. The coach will sign and have a copy on file prior to the first coaching session.
